The FSAI issued nine closure orders in March, including to Super Marios in Tullamore.
A report has found there was a risk of an overflow of foul water into the food preparation area in the restaurant, and the worker solely responsible for operating the business had not been trained in food hygiene.
Inspectors also found there was no cold water, and the hot water was at 65 degrees, which prevented thorough hand washing, the door into the chipping area was open, potentially allowing the entry of pests, and there wasn't complete and accurate documented allergen information available to consumers.
The business was closed on March 27th, and reopened last Wednesday.
